Surgical Extractions: Expert Care for Complex Cases

Most tooth extractions are straightforward — the tooth is visible, the dentist numbs the area, and it comes out in minutes. But some extractions are more involved, either because the tooth hasn’t fully erupted (as with many wisdom teeth), or because it’s broken below the gum line, or because it has unusual roots. That’s when a surgical extraction is needed.

At Spixworth Dental Practice, we carry out surgical extractions regularly, with a focus on keeping you comfortable throughout.

🧠 What Is a Surgical Extraction?

A surgical extraction is the removal of a tooth that can’t be accessed simply from above the gum line. It may involve:

  • A small incision in the gum to expose the tooth
  • Removing a small amount of surrounding bone
  • Sectioning the tooth so it can be removed in pieces
  • A few dissolvable stitches afterwards

The whole procedure is carried out under local anaesthetic, and sedation is available for nervous patients.

🔍 When Is It Needed?

  • Impacted wisdom teeth — stuck under the gum, sideways, or pressing on neighbouring teeth
  • Broken teeth — where too little crown remains to grip for a simple extraction
  • Teeth with curved, long or fused roots
  • Retained baby teeth in adults
  • Teeth damaged by infection or trauma

💬 What to Expect

  1. Assessment & X-ray — we map out the position of the tooth and any nearby nerves or sinuses
  2. Anaesthetic — the area is fully numbed; you may feel pressure but no pain
  3. Surgical removal — typically 15–45 minutes, depending on complexity
  4. Stitches & aftercare — any stitches are usually dissolvable; you’ll go home with full written aftercare instructions

🕓 Recovery

Swelling and some discomfort are normal for 2–4 days. Most people are back to normal activities within a week. Key aftercare points:

  • Bite gently on gauze as instructed to control bleeding
  • Avoid smoking, alcohol and hot drinks for 24 hours
  • Stick to soft food for the first few days
  • Don’t disturb the blood clot (no rinsing, spitting or straws on day one)

We’ll check in with you after a few days and see you for a follow-up if stitches or review are needed.

⚖️ NHS or Private

Surgical extractions that are clinically necessary are available on the NHS. Private surgical extractions are offered where you prefer faster access, longer appointment times, or sedation.
